Office Hours & Appointments
In order to maximize your rehabilitation program, regular attendance at scheduled appointments is required. If you are unable to keep your scheduled appointment, please telephone us within a 24-hour period or leave a message on our answering machine prior to your schedule time.
Insurance coverage for your therapy depends upon your individual policy. Please check with your insurance company to verify authorization or any limitations in coverage. Financial responsibility for services is ultimately yours as the patient. In order to bill any insurance company we ask that you provide us with all the necessary information to insure prompt payment. A more detailed financial policy is available upon request. Our billing office is open Monday - Friday from 8AM - 5PM to answer your questions.

The Self-Pay Rehabilitation Program
The Self-Pay Rehabilitation Program is available to those patients who require physical rehabilitation but have limited insurance coverage, no insurance coverage or insurance with a company that does not include CPW Rehab as a provider. A physician referral is required and payment for services is due at the time of treatment. Call 419-841-9622 for a fee schedule.
